Paper Forms and Documentation

Paper documentation for your own use.  Be sure to enter all Rosters and Order Forms on the https://ctc.inhs.org otherwise you will be charged a processing fee for INHS staff to enter them for you.  Do not submit two rosters.  Once you submit your roster online do not email a copy in (if you do you will be charged a second time for the emailed roster). 


BLS & Heartsaver

The AHA’s BLS courses reinforce healthcare professionals' understanding of the importance of early CPR and defibrillation, performing basic steps of CPR, relieving choking, using an AED, and the role of each link in the Chain of Survival. Courses include BLS for Heathcare Providers, Heartsaver First Aid CPR AED, and Friends and Family CPR.

Instructors who are planning on holding a BLS course must:      

  • Turn in a Complete Roster within 10 days following the class to ctc.inhs.org or (for a fee) by email, mail or fax within 10 days following the class.  
  • Turn in an evaluation per class participant. 
  • Pay for any cards that INHS is issuing for the course, pay your invoice online at pay my invoice. (Price varies depending on the type of card- look at current order form for prices)

ACLS & PALS

The ACLS course emphasizes the importance of basic life support (BLS), integration of effective BLS with ACLS interventions, and effective team interaction and communication during resuscitation.  

The PALS course pediatric-focused courses provide advanced assessment and recognition training for healthcare professionals who treat cardiovascular emergencies in infants, children, and adolescents.   

PALS- This course uses a scenario-based, team approach to teach pediatric emergency respiratory management or cardiac arrest.
PEARS- The main focus of PEARS is prevention, and specifically the assessment, recognition and stabilization of pediatric victims at risk of severe cardiopulmonary distress.
