Paper Forms and Documentation

This page contains paper documentation for your own use. Please be sure to enter all rosters and order forms at https://ctc.inhs.org; a processing fee will be charged if INHS staff must enter a roster or order form for you. Remember, you may not submit two duplicate rosters. Once you submit your roster online, please do not also email in a copy (if you do you may be charged a second time for the emailed roster). BLS & Heartsaver

The AHA’s BLS courses reinforce healthcare professionals' understanding of the importance of early CPR and defibrillation, performing basic steps of CPR, relieving choking, using an AED, and the role of each link in the Chain of Survival. Courses include BLS for Heathcare Providers, Heartsaver First Aid CPR and AED, and Friends and Family CPR.

Instructors who are planning on holding a BLS course must:      

  • Turn in a complete roster within 10 days following the class to ctc.inhs.org or (for a fee) by email, mail or fax.  
  • Turn in an evaluation per class participant. 
  • Pay for any cards that INHS is issuing for the course and pay your invoice online. Please remember that card prices vary; reference a current order form for pricing.

ACLS, PALS & PEARS

The ACLS course emphasizes the importance of basic life support (BLS), integration of effective BLS with ACLS interventions, and effective team interaction and communication during resuscitation.  

The PALS pediatric-focused courses provide advanced assessment and recognition training for healthcare professionals who treat cardiovascular emergencies in infants, children, and adolescents. PALS uses a scenario-based, team approach to teach pediatric emergency respiratory management or cardiac arrest. 

The main focus of PEARS is prevention, and specifically the assessment, recognition and stabilization of pediatric victims at risk of severe cardiopulmonary distress.

Instructors who are planning on holding a ACLS, PALS or PEARS course must:    

  • Turn in a complete roster within 10 days following the class to ctc.inhs.org or (for a fee) by email, mail or fax.
